Prenatal Visits
Congratulations on your pregnancy! It is important that you visit your OB/GYN on a regular basis. As a Cook Children's Health Plan Member you will get up to $100 for attending your prenatal visits. You can fill out this form to get started.
Find a doctor that's right for you:
During your pregnancy, you will need to visit your doctor often. Below is a table of the prenatal visits needed.

As a pregnant Member, you also have access to our Baby Steps Program.
Postpartum
Congratulations on your new baby! It is important that you visit your OB/GYN after delivery. As a Cook Children's Health Plan Member you will get a $50 gift card for one postpartum visit. You can fill out this form to get started.
After you deliver your baby, you will need to see your doctor for a postpartum visit. You will have two visits with your doctor within 60 days of the delivery of your baby. It is very important to make both appointments.
